Job Number: 23-118-11880Job Summary: The Case Worker ? Substance
Use is responsible for ensuring the provision of recovery-focused,
person-centered care with a trauma-informed perspective. This
position operates in a dynamic environment, and performs case
management, assessment, and/or service coordination work based on
the program's guidelines. The Case Worker ? Substance Abuse duties
may include and are not limited to providing continuity of care
services, linking consumers and families to all needed resources,
and coordinating, monitoring, and updating services and consumer
goals. Providing necessary skills training, assisting in crisis
prevention, and/or providing community outreach are additional
roles. As a team member of programs in substance use services, the
Caseworker ? Substance Use coordinates clinical and substance use
treatment tools to address client's needs, utilizing assessment,
multidisciplinary coordination, and efficient, clinically
appropriate utilization of agency resources. The position requires
extensive knowledge of substance use disorders and working
knowledge of treatment techniques that are effective with
chemically dependent clients, especially those who have a history
of abusing opiates, as well as a working knowledge of
methadone/buprenorphine treatment. A working knowledge of
infectious diseases such as Hepatitis, HIV, STD and other
infectious diseases is required.This position follows identified
strategies for achievement of unit performance measures and ensures
the highest quality of care while effectively utilizing
resources.This position may carry an individual caseload. On-call
responsibilities may be required dependent upon unit protocols.This
position may have additional duties associated with providing
bilingual communication services and requires the completion of a
language competency test.Regular, predictable attendance is
considered to be an essential requirement for this position. Other
duties may be assigned. May require a flexible work schedule.
