We Are Hiring:

Ascension Dell Children's is hiring for a Full-time RN Patient Navigator for our Pediatric Transplant Program! 

 

Schedule: Monday-Friday, 8am-5pm

 

As an RN Patient Navigator with the Pediatric Transplant Programs you will collaborate with a multidisciplinary team to help offer concierge patient navigation service through the continuum of the patient experience. This is a new and exciting role within a growing program at Dell Children's with an opportunity to work with luminaries in the transplant field. If you are a self-starter, highly organized, love transplant, enjoy pediatrics and collaborating with a team to save lives this may be the role for you. You will be responsible for the intake, authorizations, scheduling of transplant evaluations with the multidisciplinary team, collaborating through the transplant process, and participating in care coordination including discharge and follow up.

 

Preferred Qualifications: 

  • RN navigator experience
  • Transplant & Pediatric experience strongly preferred 
  • 2 yrs RN experience in a hospital setting

You will be able to take something that is gradual and organize it into something that grows and is functional. You will act as a community liaison and offer a level of concierge care.

 

As an RN, this role is patient facing and will require critical thinking, ability to triage, and prioritize. You will not take call, however, this will encompass the role of Transplant Coordinator during regular business hours.

What You Will Do:

RN Navigator Intake can encompass receiving patient information from referring providers and facilities including verbal report, records, diagnostic tests, and studies. You will be collaborating with multiple disciplines to ensure that the medical information from the referral is placed into our EHR in a timely manner. This role will be responsible for overseeing all medical records from the outside, and may participate in the clerical aspects that accompany patient medical records. Verification of receipt and timely placement of medical records within the EHR is a critical aspect of this role. For our RN navigator it will be important to have the clinical knowledge to be able to triage the medical history and staff higher acuity patients with the provider to determine timing of care delivery. The RN navigator role is responsible for patient and insurance facing transplant financial aspects of the Pediatric Transplant programs. This may include creating patient accounts, and verifying and obtaining insurance authorization for office visits, medications, and hospitalizations. A key aspect of this role will be to function in the role of the Transplant Financial Specialist to obtain financial authorization for transplant evaluation, VAD placement, and transplant listing. This includes meeting patients and families and discussing their insurance coverage with accurate and timely documentation. The RN navigator will be able to organize the patient transplant volume, track new referrals and established patients, and have a pulse on patient follow up (new and established patients). The RN Navigator will be keenly aware of what is required from each payer to evaluate, list, and maintain listing. Additionally, this role will build relationships with our payer partners to allow for streamlined care. Our RN Navigator will assist in scheduling speciality clinics, schedule and participate in discharge teaching of the post transplant patient, participate in writing the letters of medical necessity and setting up peer to peers as needed, (referring provider letters and documentation) and participate in lists and patient follow up statistics.

 

Responsibilities:

  • Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinate efforts for treatment and promotes quality improvement and educational efforts.
  • Compile data, track outcomes, and make recommendations for process improvement.
  • Provide referrals/support for follow-up care for patients who are uninsured or under-insured.
  • Intake, Authorizations, Transplant Financial, Discharge follow up, Referring provider point of contact, and liaison for sub-specialists.
What You Will Need:

Licensure / Certification / Registration:

  • Required Credential(s):
    • Licensed Registered Nurse credentialed from the Texas Board of Nursing.
  • Preferred Credential(s):
    • BLS Provider credentialed from the American Heart Association (AHA).

Education:

  • Diploma from an accredited school/college of nursing required.
