POSITION SUMMARY The Disease Management Coordinator provides leadership and promotes excellence in patient care of cardiac, neurologic, and other disease conditions in the Emergency Department, ICU, Medical Surgical and Rehabilitation Units, along with other entities within the healthcare team. This position is responsible for planning, development, and implementation of program goals and operations including Primary Stroke Center Designation. The Disease Management Coordinator also coordinates education of staff and physicians, oversees the ongoing quality improvement process to improve patient care. This position ensures compliance with the accreditation standards and collects and reports data to external agencies (as required) and internal committees. ESSENTIAL JOB RESPONSIBILITIES Concurrent Review/Data Abstraction/Data Reporting Performs daily concurrent chart review to ensure compliance with evidence based program measures for Stroke and AMI. Retrospectively abstract Stroke and AMI data from the medical record (HPF, Meditech, etc.) and enter data into the appropriate vendor database (Comet, NCDR, etc). Perform Inter-Rater Reliability chart review. Ensures all data reporting timelines are met (IRR, normalization, etc). Collaborate with clinical staff, physicians, and leaders to ensure evidence based measures are met in a timely manner. Implements and revises the required policies, procedures, and protocols for Stroke Services and Cardiac Care. Initiate patient transitions thru departments, and movement of patients to other levels of care; i.e. rehab (as needed). Perform patient/family education on Stroke and Cardiac Care (as needed). Facilitate Performance Improvement Activities Serves as a resource to physicians, nurses, educators, and leaders on the evidenced based measures for Stroke and AMI. Identifies educational and process improvement opportunities that support compliance with program measures. Coordinates with nursing staff and hospital leaders to establish and implement standardized, evidenced based processes/protocols. Analyzes, trends, and reports data to various hospital and regional committees (Quality Committee, Stroke Committee, CATRAC Subcommittees, etc.). Chairs the hospital Stroke Committee. Represents the hospital at a local, regional, and national level related to best practices for Acute Coronary Syndrome and Stroke patients. Maintain regulatory compliance with accreditation standards Maintains knowledge and understanding of Stroke accreditation requirements. Compiles and submits accreditation program application, performance improvement plan, and measures of success as required. Performs tracers to evaluate hospital compliance with regulatory standards. General Duties Attends and actively participates in monthly staff meetings, and attends called departmental meetings when necessary. Provides community education opportunities to promote health awareness in Stroke and Cardiac prevention. Attends and participates in facility committees, employee forums and departmental meetings as requested. Actively utilizes and complies with facility principles of good communication and customer service standards, including use of AIDET and KWAKT as developed by the department. Maintains compliance with required licensure, ethics and compliance training, annual mandatory TB screening and mandatory education as required. Qualifications EDUCATION AND EXPERIENCE Required: Graduate of accredited nursing school with Nursing Diploma, Associates Degree or higher level education. Minimum of three years clinical experience. Preferred: Bachelor of Science in Nursing. Documented experience demonstrating leadership, management, communication, and interpersonal skills. LICENSES AND CERTIFICATES Required: Texas RN License or a Compact RN license from a NCLA Compact State REPORTING RELATIONSHIP Reports to: Director of Quality
