Description SHIFT: Nights (rotating weekends) SCHEDULE: Full-time St. David's Round Rock Medical Center is part of St. David's HealthCare , one of the largest health systems in Texas, which was recognized with a Malcolm Baldrige National Quality Award in 2014. With 171 beds, the hospital's comprehensive services include advanced cardiac care at the Heart Hospital of Austin at St. David's Round Rock-the first specialized cardiac facility in Williamson County-with the only Hybrid Operating Room (OR) in the county; as well as emergency services in a 24-hour emergency department, including advanced trauma care with a Level II Trauma Center; St. David's Urgent Care Round Rock; and St. David's Emergency Center in Cedar Park. St. David's Round Rock Medical Center is also known for providing advanced orthopedic and robotic services, among other complex specialties. In 2016, the hospital received the Distinguished Hospital Award for Clinical Excellence from Healthgrades and also earned a national distinction for patient safety from The Leapfrog Group . POSITION SUMMARY This position, under the supervision of the Supervisor or Charge Nurse coordinates and provides patient care. This care includes attending deliveries to advocate for the needs of the newborn(s) after delivery. Assesses and assists in safe extrauterine life adjustments of the newborn. Allows skin-to-skin (STS) bonding while monitoring the stability of the newborn(s) thermoregulation, any anomalies or signs of distress. Requires proficient resuscitation skills. Works in conjunction with labor and delivery nurses, obstetricians, and pediatric/neonatology staff. Completes the initial head-to-toe assessment, VS, medication administration, and any ongoing assessments of the newborn and mother during the two-hour transition period in accordance to policy and guidelines of Round Rock Medical Center. Development of an individualized patient care plan, family centered patient teaching, the safety identification of the newborn, facilitating breastfeeding (as applicable) May be asked to assist with circumcision or task in L&D or M/B units. Staffing ratios are in accordance with AWHONN guidelines. This position requires having two or more skill sets in order to assist other areas when deliveries are not imminent. The secondary skill set(s) choices include L&D, M/B, or NICU. The secondary skill set(s) job description will be in accordance to those units. ESSENTIAL JOB RESPONSIBILITIES Assigns initial Apgars and facilitates skin-to-skin bonding for a minimum of 1 hour Assists with breastfeeding or feeding choices during the first hour and beyond prior to moving to M/B. Assesses and re-assesses the newborn(s) respiratory and airway support using neonatal resuscitation program guidelines, thermoregulation, and metabolic stability as appropriate. Develops care plans that promote, facilitate, and maintain newborn wellness. Provides newborn care at the bedside while promoting skin to skin, breastfeeding, bonding and transition support. Administers medications as needed, provides neonatal abstinence scoring and urine/meconium collection as needed, as well as, blood glucose and Sa02 monitoring, lab work. Implements appropriate patient care interventions, and evaluates patient care interventions to ensure appropriate level of care is provided. Communicates effectively with other departments in an effort to resolve issues ensuring efficient and optimal patient care outcomes. Acts as mentor to Labor and Delivery & Mother-baby Nurses as the relief to transition care during busier times. Ensures that physician orders for the NB are noted and correctly transcribed. Promotes a positive working environment. During adoption follows maternal requests for STS and bonding. Follows the guide of the delivering mom in allowing the adoptive parents the same experience. Surrogacy adoption is also given the opportunity for STS bonding. Plans comprehensive nursing care for assigned patients according to written hospital standards and physician orders. Nurses notes reflect individualized plan of care with each assessment. Reviews plan of care prior to transfer to M/B or NICU in the 2 hours of transition care. Appropriate intervention and nursing care must be documented. Initiates interdisciplinary admission planning by identifying needs, assisting with teaching, facilitating the coordination of family with other services. P rovides and documents nursing care and interventions for assigned patients in compliance with written hospital policies and procedures and according to physician's orders in a competent, safe and timely manner. Accurately administers documents medications within 2 hours of delivery. Documents any delay or declination of medications if required by the state of Texas. Provides individualized nursing interventions reflecting current nursing practice, based on plan of care as patient's condition warrants. Initiates appropriate teaching and / or referrals. Patient Care: Responds to the developmental needs of the patient and family by demonstrating basic knowledge of patient growth and development, and by providing individualized patient/family education, which addresses language, culture, and learning, needs. Keeps physicians and the supervisor and/or Charge nurse aware of significant information regarding the patient and family. Collaborates with and seeks assistance from other health care professionals as needed. Evaluates patient care outcomes in an appropriate and timely manner Evaluates plan of care to determine need for modifications in a timely and prompt manner. Evaluates effectiveness of patient teaching and revises teaching plan as indicated. Evaluation is systematic and ongoing with patients responses to interventions and documented. The patient, mother or adoptive parents, significant other and health care providers are involved in the evaluation process, when possible. Maintains current professional nursing knowledge. Provides evidence of mandatory education requirements per hospital policy and continuing education relevant to the clinical practice area as regulated by the board of nursing for the state of Texas. Completes unit specific competency checklist and age specific competency requirements. Completes competencies on hyperbilirubinemia, hypoglycemia, thermoregulation, and Hugs system. Looks to be on a hospital or unit committee Qualifications EDUCATION AND EXPERIENCE Required: Graduation from a NLN accredited school of nursing as an RN One-year experience in Newborn, M/B, NICU or NB Transition care setting. Preferred: BSN LICENSES AND CERTIFICATES Required: Licensed in the state of Texas as RN A HA BLS at time of hire NRP at time of hire. Second skill set, can include L&D, M/B, and/or NICU Preferred: Certification in area of specialty S.T.A.B.L.E Certification
