St. David's South Austin Medical Center is part of St. David's HealthCare, one of the largest health systems in Texas, which was recognized with a Malcolm Baldrige National Quality Award in 2014. St. David's South Austin Medical Center is an acute care facility with 316 beds, offering a range of complex specialties and sub-specialties, including a nationally accredited oncology program with the area's only adult bone marrow transplant program; a trauma program that includes all of the capabilities and programmatic elements of a Level II trauma center to treat the most severely injured and critical patients; a comprehensive cardiac program; full-service maternity and newborn care with Level I and II nurseries; and two full-service emergency centers in the communities of Bee Cave and Bastrop. POSITION SUMMARY The Sepsis Coordinator is a health care professional who works collaboratively with the medical and hospital staff to improve outcomes in the Septic patient population. This position provides guidance regarding CMS and HCA initiatives surrounding sepsis care; collects and abstracts data from charts; translates the data into actionable information; drives the performance improvement process; formulates committee reports; and, works collaboratively with other program coordinators to standardize processes. Job Specific: The Sepsis Coordinator performs concurrent and retrospective chart reviews, collects and analyzes data, implements improvements to reduce variation in care, formulates committee reports, works with Division and Facility leadership to identify improvement opportunities, and continually reevaluates the care of the acutely ill septic patient. Prioritizes activities that promotes the Sepsis Program within the facility. The Sepsis Coordinator is responsible to ensure that bedside nursing staff understand the need for education of sepsis patients and family. To coordinate the efforts of the hospital in order to meet the needs of the patients and the customers. To improve the quality of patient care by educational and QI programs. ESSENTIAL JOB RESPONSIBILITIES* Individual Leadership: Demonstrates confidence in own skills, knowledge and ability and opportunities for personal growth and ability to accept and adapt to change. * Customer Service: Demonstrates customer service skills with both internal and external customers. * Transcultural Sensitivity: Treats patients/customers as individuals with unique values, beliefs, and practices. * Risk Management/Patient/Customer Safety: Demonstrates the ability to mitigate risk and support patient/customer safety and according to policies and procedures. * Educator: Identifies, implements, and/or evaluates opportunities for patient/family/physician/community education. * Infection Control: Demonstrates infection prevention and control practices congruent with facility policies and procedures. * Performance Improvement: Performs, participates, implements and evaluates performance improvement activities at the department, facility and division level. * Financial Accountability: Identifies opportunities for waste reduction and process efficiencies. * Information Management: Demonstrates adherence to information management policies and procedures, maintaining confidentiality of PHI and PPI at all times. * Communication: Works as a team member through professional interactions, demonstration of appropriate communication skills and reporting/activation of the chain of command. * Practices Professionalism: Performs, participates, implements and evaluates personal performance in light of the ANA Code of Ethics and Texas Board of Nursing requirements for professional nurses. * Ethics/Compliance: Performs all duties with integrity, behaves ethically, complies with all policies and procedures, federal, state, and local laws. * Environment of care: Demonstrates accountability in role and responsibilities. The Coordinator provides clinical expertise for members of the medical staff, nursing staff, patient, families and care givers. Scope of practice includes supervising the care provided to the patients with sepsis, focusing on health promotion and disease prevention.* The Coordinator also functions as an educator and coordinates care through the continuum ensuring optimal patient care and outcomes. * The Coordinator will collect and analyze data from those patients who have been identified with sepsis, assist with documentation, and clarify coding and registration issues. * Develops, reviews and revise(s) the sepsis protocol to become the standard of care within the facility. * Providing leadership and participating in the quality improvement process. * Applies verbal and non-verbal skills to enhance patient/customer satisfaction. * Collaborates with all members of the interdisciplinary team to enhance best practices. * Maintains active involvement in hospital committees. * Coordinator will lead the Sepsis Committee, maintain agenda and complete the related administrative duties as necessary, including creation of meeting packet. * Attends sepsis calls and division meetings. * Daily reviews of in-house patient charts in real time to provide feedback to care team members. * Employee's conduct must reflect the Company's values and a commitment to the Code of Conduct Ethics and Compliance Program. EDUCATION AND EXPERIENCE Required: 3-5 yrs. critical care experience required; BSN required. Preferred: MSN preferred LICENSES AND CERTIFICATES Required: Registered Nurse license by the Texas Board of Nurse Examiners. If compact license held, RN is required to obtain a Texas RN license within 90 days of their hire date or the ability to obtain and BLS Preferred: ACLS, PALS REPORTING RELATIONSHIP Reports to: Director of Quality
