St. David---s North Austin Medical Center is part of St. David---s HealthCare, one of the largest health systems in Texas, which was recognized with a Malcolm Baldrige National Quality Award in 2014. The 378-bed multi-specialty, acute care facility is dedicated to the highest level of women---s health services, including maternity and newborn care with Level I, II and III nurseries at the adjacent St. David---s Women---s Center of Texas. The facility also features a 24-hour emergency department, the Texas Institute for Robotic Surgery, the Bariatric Center, heart and vascular center, neurology and neurosurgery, a kidney transplant program, inpatient and outpatient surgery, and acute inpatient and outpatient rehabilitation, among many others. St. David---s North Austin Medical Center is also home to St. David---s Children---s Hospital. In 2012, 2013, 2015 and 2016, the hospital earned a national distinction for patient safety from The Leapfrog Group.

Working cooperatively as a member of the transplant team, the transplant coordinator is responsible for coordinating all the activities related to pre- and post-transplant patients. These activities include evaluating potential recipients and living donors; educating potential recipients, living donors, and post-transplant patients; providing consultative services and education to nursing services during the transplant recipient---s hospitalization; and coordinating follow-up care of the transplant patient. The St. David---s HealthCare Registered Nurse (SDHRN) fulfills an important role as a central patient care provider responsible for the coordination and delivery of patient centered nursing care. As a full partner of the healthcare team, the SDHRN collaborates with all other healthcare disciplines to achieve successful execution of the patient---s plan-of-care. The SDHRN utilizes physical assessment skills and advanced diagnostic technology to aid in the assessment of the patient---s nursing care needs. Data collected is analyzed, synthesized and disseminated to other healthcare providers. The SDHRN continues to monitor and assess the patient---s physical condition, emotional and social needs, continually refining the plan-of-care to ensure achievement of the desired goals of the patient and facility outcomes.

Participates in providing patient specific care standards as directed, and follows service excellence standards to ensure high levels of patient satisfaction.


Note: Job responsibilities are divided into pre- and post-transplant and coordinators are hired to focus on one primary role; however, coordinators are cross-trained in both areas of transplant care for coverage purposes.


Coordinates assessment and evaluation of potential transplant recipient and living donor.

  • Prioritizes data collection according to the patient---s condition or needs.

  • Obtains physician approval for the patient as a transplant candidate.

  • Provides data coordinator with all information necessary to meet UNOS and Medicare regulations.

  • Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs.

  • Assists in development of Long Term Care Plans for each patient.

  • Coordinates all surgical arrangements for living related donor transplants.

  • Takes for all donor offers according to the monthly call schedule, as well as post patient after hours calls.
    Provides education about transplantation to patients and their families. This education includes:

    • Evaluation process/UNOS listing process

    • Risks and complications of transplantation

    • Hospital admission/surgical procedure

    • Medication: effects, side effects, dosing, interaction with food

    • Maintenance of diary

    • Self-care at home: vital signs, intake and output, weight

    • Follow-up appointments

    • Prevention of infection

      Assess potential learning problems (language barrier, readiness to learn, etc.) and structures teaching sessions to meet individual needs.

      Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs.


      Assists nursing units in the care of all transplant patients to ensure proper admission and care of the hospitalized patient.

      • Reviews charts, test results and medications.

      • Makes rounds on hospitalized patients.

      • Communicates issues to physicians.

      • Provides discharge teaching education to patients and family members.

      • Assists in arranging discharge medications.
        Coordinates and assures thorough and consistent follow-up care of the transplant recipient with the nephrologists, transplant surgeons, and other care providers.

        • Compiles and reviews data prior to clinic visits.

        • Follows the transplant recipient in the transplant clinic.

        • Collects and evaluates outstanding data after each visit.

        • Provides data coordinator with all information necessary to met UNOS and Medicare regulations.
          General Responsibilities
          Educates staff and community in the area of transplantation.
          Rotates 24-hour call duty for donor screening, coordination of transplant team for deceased donor transplants, and triage of patient calls.

          Required: Graduate from an accredited nursing program or current enrollment in an
          Alternative Entry MSN program; Minimum of two years clinical nursing experience

          Preferred: One year critical care nursing experience; experience with renal transplantation or
          dialysis; Bachelor---s or higher level Degree

          Required: Texas RN License or a Compact RN license from a NCLA Compact State; BCLS Certification

          Preferred: ACLS; Certified Clinical Transplant Coordinator
