RN - Case Management Contract Position in Austin, TX Location
Austin, TX Assignment Duration This is a 13-week contract position.
Schedule Monday - Friday, 8:00 AM - 5:00 PM Job Description We are
seeking two experienced Registered Nurses for a Case Management
position. The RN Case Manager will facilitate discharge planning
and engage in difficult conversations with patients, families, and
medical teams to establish and implement effective care plans.
Responsibilities Conduct discharge planning and progression. Engage
in hard conversations with families and patients. Collaborate with
medical staff to discuss and plan patient care. Required Experience
/ Certifications / Licensure At least 1 year of bedside case
management experience. Knowledge of post-acute care. Experience
