Job Description:
Ascension is seeking a compassionate and dedicated Registered
Nurse Patient Navigator to join our Orthopedic Medical Unit. As

personalized care. In this role, you will guide patients through
the healthcare system, ensuring seamless, coordinated care and
empowering them with the information needed to manage their health.
This position requires strong interpersonal skills and a commitment
to improving patient outcomes in the orthopedic department.
Coordinate comprehensive care plans for patients in the orthopedic
unit. Act as a primary point of contact for patients, guiding them
through the healthcare process. Educate patients and their families
on treatment plans and healthcare resources. Collaborate with
multidisciplinary teams to enhance patient care quality and
efficiency. Monitor patient progress and adjust care plans as
necessary. Facilitate access to necessary medical services
including scheduling, transportation, and access to specialists.
Advocate for patients' needs and preferences with healthcare
providers and staff. Strong clinical skills in orthopedics.
Excellent communication and interpersonal skills. Ability to work
collaboratively in a multidisciplinary health team. Strong
organizational and problem-solving skills. Proficiency in
electronic health record (EHR) systems. Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Active Registered
Nurse (RN) license in the state of employment. Certified Nurse in
Orthopedics (ONC) preferred. Basic Life Support (BLS) certification
required.
