Job Description Job Description The HCS Case Manager is
responsible for coordinating and overseeing services for
individuals enrolled in the Home and Community-Based Services (HCS)
program. This position ensures compliance with Texas Health and
Human Services Commission (HHSC) regulations, supports
person-centered planning, and promotes the health, safety, and
well-being of individuals served by Just Like Home. The HCS Case
Manager serves as a liaison between individuals, guardians, service
providers, and regulatory agencies to ensure services are delivered
in accordance with the Individual Plan of Care (IPC) and
Person-Directed Plan (PDP). Essential Duties & Responsibilities
Coordinate and monitor all HCS services to ensure compliance with
HHSC regulations. Facilitate and document Person-Directed Planning
(PDP) meetings. Develop, review, and update Individual Plans of
Care (IPC). Conduct monthly face-to-face visits with individuals
served (or as required by regulation). Ensure implementation of
service goals and monitor progress. Maintain accurate, audit-ready
documentation in accordance with HCS and Medicaid standards. Review
incident reports and ensure proper follow-up and corrective action.
Coordinate medical, behavioral, therapeutic, and ancillary
services. Assist individuals and guardians with advocacy, benefits,
and community resource connections. Ensure compliance with rights,
dignity, and protection of individuals served. Participate in
interdisciplinary team meetings. Monitor staff performance related
to implementation of service plans. Assist with HHSC surveys,
audits, and corrective action plans. Maintain confidentiality in
accordance with HIPAA and state regulations. Qualifications
Bachelor’s degree in Social Work, Human Services, Psychology,
Special Education, or related field (preferred; experience may
substitute if permitted by regulation). Minimum one (1) year
experience working with individuals with intellectual or
developmental disabilities (preferred). Knowledge of Texas HCS
waiver program rules and Medicaid requirements. Strong
documentation and organizational skills. Ability to work
independently and manage multiple caseloads. Valid Texas driver’s
license and reliable transportation. Knowledge, Skills & Abilities
Strong understanding of person-centered planning principles.
Ability to interpret and apply HHSC regulations. Excellent
communication and advocacy skills. Strong critical thinking and
problem-solving abilities. Proficiency with electronic
documentation systems. Ability to maintain professional boundaries
and confidentiality. Working Conditions Combination of office,
group home, and community settings. Frequent travel within assigned
service area. May require flexible hours to accommodate individual
meetings. Physical Requirements Ability to sit, stand, and travel
for extended periods. Ability to lift up to 25 pounds if necessary.
Ability to safely navigate community environments.
