Job Title: RN - Case Management Profession: RN Specialty: Case Management Duration: 13 Weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum 1 Year License: RN License Required Certifications: None specified Description: This is a contract position for an RN specializing in case management. The role requires strong communication skills for engaging in difficult conversations with patients and their families. Experience in discharge planning and assessing patient needs is essential. Candidates must have a good understanding of post-acute care and familiarity with medical software. There are no on-call requirements for this position. Rotating weekend shifts may be required on an every other basis. Ideal candidates should be adaptable and proactive. Dress code includes navy scrubs paired with a white lab coat and closed-toe shoes.
