We can't wait to welcome you to the Strive Team!*JOB DUTIES** Assess home health pediatric patient for rehabilitative and functional status.* Regularly re-evaluate the patients/clients physical therapy need and evaluate the outcomes of care on a regular basis.* Physical Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.* Identifies deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.* Physical Therapist evaluates the need for and recommends DME devices and aides.* Uses infection control measures that protect both the staff and the patient (OSHA).* Coordinates services. Informs the physician and other personnel of changes in the patient's/clients condition and needs.* Physical Therapist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.* Supervises, teaches, and provides clinical direction to other physical therapy personnel as required or requested.* Promote Agency philosophy and administrative policies to ensure quality of care.* Physical Therapist provides effective communication to patients/clients, their family members, team members, and other health care professionals.*QUALIFICATIONS** Graduate of an accredited Diploma, with Bachelors, Masters, or Doctorate degree* One year of experience as a Physical Therapist in a pediatric clinical care setting preferred* Pediatric Home health experience preferred* Reliable transportation* Current Texas State license as a Physical TherapistWe look forward to discussing your future career as a part of the Strive Health Services family!Job Type: Part-timePay: $90.00 per hourBenefits:* Continuing education credits* Flexible schedule* Opportunities for advancement* Travel reimbursementMedical Specialty:* Home Health* Pediatrics* Physical & Rehabilitation MedicineSchedule:* Monday to FridayLicense/Certification:* Driver's License (Required)* Physical Therapy License (Required)Work Location: In personby Jobble
