*HealthMark Rehabilitation *is an _inhouse_ therapy company, serving the Skilled Nursing Facility patients and residents of *HMG* facilities.Were currently seeking a *Full-time & PRN Physical Therapist* to evaluate and treat residents and short-term patients at our *Accel at College Station* facility in *College Station, Texas.**1500 Medical Avenue, College Station, TX 77845*****$5,000 SIGN ON BONUS*** (FULL-TIME)** *Prefer DPT candidates but non-DPT will be considered** *PRN not eligible for Sign on bonus**Responsibilities *include: Develop and implement the Physical Therapy plan of care for facility residents and short-term patients. Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care. Assist as needed with patient screenings for the need for Physical Therapy. Communicate progress and any barriers toward PT goals with interdisciplinary team members. Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements. Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods. Provide and document patient, family, and caregiver training regarding each PT patients specific plan of care and recommendations for follow up. Perform additional duties as required.*Qualifications *include*:* PT degree from an accredited Physical Therapy program. Current Texas license in good standing as a Physical Therapist. Meet physical requirements to deliver PT responsibilities. Experience in a geriatric rehabilitation setting preferred. The first payment of $1,000 will be made with the next payroll after the full-time employee as documented above has successfully completed 90 days of work without any disciplinary action. The second payment of $1,000 will be made with the payroll following the completion of 6 months of satisfactory work performance at full-time status. The third payment of $1,000 will be paid upon the successful completion of 9 months of full-time employment without any disciplinary action.
