Job SummaryWe are seeking a skilled RN-Patient Navigator to join our team at Ascension Seton Northwest. As a key member of our healthcare team, you will play a vital role in coordinating patient care and promoting quality improvement initiatives.Responsibilities

• Act as a liaison between management staff, administration, physicians, and other stakeholders to ensure seamless patient care.
• Coordinate treatment efforts and promote quality improvement initiatives to enhance patient outcomes.
• Compile data and track outcomes to inform process improvements and make recommendations for enhanced patient care.
• Provide referrals and support for follow-up care for patients who are uninsured or underinsured.Requirements
  • Registered Nurse credentialed from the Texas Board of Nursing required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.Preferred Qualifications
    • Case management experience
    • Pediatric experience preferred
