    Job Summary and QualificationsAs a Palliative Care Nurse Coordinator, you will be responsible for the development and implementation of a palliative care program. You will provide guidance, support, and education to healthcare team members in the overall delivery of services related to palliative care.
    • You will establish a palliative care plan for all palliative care patients based upon assessments, patient care standards, and patient and family preferences
    • You will consult and collaborate with healthcare team members to develop a plan of care and palliative care recommendations
    • You will ensure consistent, complete, and thorough nursing documentation
    • You will review physician orders related to palliative care, and write or enter orders into the system accurately
    • You will provide timely communication to nurses and/or physicians regarding changes to the plan of care and/or changes in the patient's condition
    • If necessary, you will provide basic life support in collaboration with the healthcare team
    • You will maintain and review patient records, charts, and documents for accuracy, quality, and best practices related to palliative care
    • You will provide palliative care data, including the number of consults, referrals, conversions to hospice, and changes in level of careJob RequirementsExperience
      • 1 year of clinical nursing experience as a Registered Nurse (RN)
      • Previous experience in Hospice and Palliative Care nursing
      • Critical care experience in a hospital setting - preferredEducation
        • Associate's degree in Nursing (ADN)
        • Bachelor of Science in Nursing (BSN) - preferred
        • Advanced Practice Nurse (APN) - preferredLicenses and Certifications
          • Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, Texas license must be obtained within 90 days of hire
          • Basic Life Support (BLS) certification as per St. David's Healthcare policy
          • Certified Palliative and Hospice Care Nurse (CHPN) - preferred
