OverviewWe are looking for a Registered Nurse PC to join our team. - This position will directly report to the Assistant Director PC, Branch Director PC or Branch Manager PC and is responsible for providing -skilled nursing care including skilled assessment and instruction in the home under a plan of care/treatment.

• Confers with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provides hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. - Revises plan in consultation with physician based on ongoing assessments and as required by policy/regulation
• Admits patients to the services, completing appropriate clinical documentation under the Clinical Manager's direction. Assesses individual status and care/instruction needed by the patient
• Coordinates/oversees/supervises the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitors the appropriate completion of documents.About You
  • Bachelors / Associates degree in Nursing or equivalent.
  • RN licensure
  • Proof of current CPR certification.
  • Six months' of professional nursing as a Registered Nurse. -We Offer
