Join a mission with a company behind it.**This role requires Spanish fluency.****What You'll Be Doing:**+ Connecting with the patients in person, on the phone, embedded (on site) in the physician offices or in the home, providing face to-face interaction with patients and their care team when appropriate to improve patient care, primarily in hospitals/facilities and through home visits+ Along with other members of the population health team, conducting comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identifying gaps in care and barriers to achieving improved health; facilitating and collaborating with the facility Utilization Management, Case Managers, Discharge Planners and/or all appropriate ancillary providers+ Based on this assessment and in conjunction with the patient, the patient's physician and other members of the population health team, creating and implementing a care plan that will address the identified needs, remove the barriers and improve the health of the patient+ Coordinating and collaborating care by serving as the contact, advocate and resource for the patient, their family, and their physician(s), building effective relationships through trust, respect, and communication+ In close collaboration with the patient, primary care provider and care team, continually assessing the patient's knowledge of their clinical condition(s) and providing education and self-management support based on the patient's unique learning style+ Measuring, improving and maintaining quality outcomes (clinical, financial, and functional) for individual patients and the population served+ Collaborating across the ECP team and participate in clinical operations initiatives as needed+ Driving improvement in patient outcomes across multiple geographic regions**The Experience You'll Need (Required):**+ Associates Degree in Nursing+ 3-5 years of nursing experience, preferably in home health, ambulatory care, community or public health, case management, coordinating care across multiple settings and with multiple providers+ A current North Carolina, Texas, Indiana, and/or Michigan Registered Nurse License, or compact state licensure+ Fluency in Spanish**Finishing Touches (Preferred):**+ Bachelor's Degree in Nursing+ Certification in Case Management through Commission for Case Manager Certification (CCMC)+ We are looking for team members who possess enthusiasm, a strong work ethic, and integrity, who are kind and empathetic, have a sense of humor, and who enjoy working in a collaborative, team-oriented system+ Knowledge and experience teaching self-management+ Exceptional communication skills, both written and oral, and the ability to positively influence others with respect and compassion+ Strong work ethic built on a foundation of proactivity and teamwork+ Ability to navigate ambiguity with the aid of structured problem-solving techniques+ Committed to the practice of inquiry and listening+ Experience with and embraces utilization of electronic health records+ Interest in being a part of a diverse and collaborative start-up team**Technical requirements:**During the current pandemic Evolent employees are working remotely from home. As such we require that all employees have the following technical capability at their home: High speed internet over 10 MBPS and, specifically for all call center employees, the ability to plug in directly to the home internet router.
