In FY2018, Ascension provided nearly $2 billion in care of persons living in poverty and other community benefit programs.What You Will Do As an RN Coordinator in Advance Heart Failure with Ascension Seton Family of Hospitals-TX, you will have the opportunity to coordinates nursing program activities and resources within clinical specialty or department. In this role, you will be responsible for outpatient care coordination for advance heart failure clinic. We offer the highest level of care and amazing teamwork. We have nurses who have been with our group and program for 10-20 years. We work hard, love what we do and who we do it for. This is an amazing opportunity to be a part of something great. We are looking for someone with heart failure/VAD/heart transplant coordinator experience to take on this role.Responsibilities:

  • Develops and oversees program, making recommendations and acting as a resource for all nursing staff related to patient care and clinical standards, policies and procedures.
  • Develops and documents care plans and therapy programs.
  • Oversees and ensures plan of care therapy program is implemented as indicated.
  • Ensures necessary resources and/or equipment is obtained as ordered.
  • May supervise and direct nursing staff.
  • Coordinates, assembles, and maintains documentation as required.What You Will Need Licenses/Certifications/Registration:
    • Required Credential(s):
      • BLS Provider credentialed from the American Heart Association (AHA) obtained within 3 months (90 days) of hire date or job transfer date.
      • Registered Nurse credentialed from the Texas Board of Nursing. Education:
        • Diploma
          • Diploma from an accredited school/college of nursing required. Work Experience:
            • 5-10 years of experience in advanced heart failure/VAD/heart transplant strongly preferred.
