We Are Hiring RN-Patient Navigator - SFC Hem Onc DCMC Support - Full-Time,Day - Strictly Peds Medical Office Building - Austin,TXSchedule: M-F 8:30am to 5pmLocation: Children's Blood and Cancer Center -Why Join Ascension? In FY2018, Ascension provided nearly $2 billion in care of persons living in poverty and other community benefit programs. -What You Will Do As a RN Patient Navigator -with Seton Family of Hospitals-TX, you will have the opportunity to serve -as a patient navigator throughout the continuum of this role, you will coordinate for the Pediatric Neuro-Oncology program. You will manage the weekly neuro-oncology conference, clinic and various team meetings. New patient education and coordination of Neuro Oncology services. This role, provides opportunity to work very closely with neurosurgery team, neuro oncology team with various interesting cases. This role also provides an opportunity to help patients and families by educating them and help them maneuver a very complicated system and is the main point of contact for the neuro oncology program.Responsibilities:Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.Coordinates efforts for treatment and promotes quality improvement and educational efforts.Compiles data, tracks outcomes, and makes recommendations for process improvement.Provides referrals/support for follow-up care for patients who are uninsured or underinsured.What You Will Need Licenses/Certifications/Registration:Required Credential(s):Registered Nurse -credentialed from the -Texas Board of Nursing -. - -Preferred Credential(s):BLS Provider -credentialed from the -American Heart Association (AHA). -Education:Diploma -Diploma from an accredited school/college of nursing requiredBachelor's in Nursing Preferred.Work Experience:Pediatric Nursing, Pediatric Oncology Nursing Preferred.None requiredEqual Employment Opportunity Seton Healthcare Family is an equal employment opportunity / Affirmative Action employer and complies with all applicable federal, state and local equal employment regulations and laws.
