We Are Hiring RN-Patient Navigator - Dell Children's Medical Group Hearing Program - Full-Time, Day - Strictly Pediatrics Medical Office Building - Austin, TXThe Hearing Center at Dell Children's (THC) under (ENT) Cochlear Implant (CI) Dr. Corrie RoehmSchedule: Monday through Friday 8-5Location: Strictly Pediatric Office ?Why Join Ascension? In FY2018, Ascension provided nearly $2 billion in care of persons living in poverty and other community benefit programs.?What You Will Do As the RN Patient Navigator you,?will navigate the patients/parents through the healthcare network as a cochlear implant candidate, they see Dr. Roehm, audiologist, speech language therapy before and after the implant. This RN will track all appointments for Neuro Psychic, MRIs, labs and office THC visits etc. as needed to proceed through the candidacy pathway. When the candidate is selected for surgery, on surgery day the RN will go over to DCMC and sit with parents for a few minutes to answer questions and to just be there if needed. The RN will be responsible for many patients at once and will need to be aware of where the child is in CI candidacy and after care process/and progress. RN will organically discover barriers to success of treatment it will be important to be a great communicator with Social Work, parents/patients and CI team for those barriers to be removed by assisting Social Worker. Will work directly with Dr. Roehm's clinical RN daily/weekly for communication on behave of CI patients.?Must have a?Huge heart full of compassion for children with special needs and understanding and empathy of grief process of parents, able to walk beside them in this process. Be a strong organizational person with an understanding of technology is a must?you track all aspects of the patients care and document in EHR and CI data base. Must be a strong leader and work well within a team approach to healthcare with multiple service providers, community providers, partners ISD's etc. Must be a clear communicator and have follow through to close loops for the patients care as the RN navigates the patient through the CI candidacy road map.Responsibilities:

  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.What You Will Need Licenses/Certifications/Registration:
    • Required Credential(s):
    • Registered Nurse?credentialed from the?Texas Board of Nursing.??
    • Preferred Credential(s):
    • BLS Provider?credentialed from the?American Heart Association (AHA).?Education:
      • Diploma?
      • Diploma from an accredited school/college of nursing required. Work Experience:
        • ENT experience preferred.
        • Good knowledge base/understanding of the hearing-impaired world preferred.
