for additional specifics.ResponsibilitiesAscension is currently
seeking an Obstetrics Case Manager - Registered Nurse (RN) to
support programs of Dell Children's Health Plan (DCHP). A s a Case
Management Registered Nurse wtih a Compact Texas License, you will
coordinate the overall interdisciplinary plan of care for patient,
from admission to discharge. Other responsibilities include:
- Responsible for telephonically and/or face to face assessing,
planning, implementing and coordinating all care navigation
activities with members
- Establish care plan, goals, interventions, and contact schedule
based on risk category, and patients/family members identified
medical and social needs
- Continually monitor and document progress and new developments
with respect to care plan goals
- Promote compliance with disease specific clinical outcomes by
providing each member with self-management supports including
disease specific education materials
- Coordinate care and communication between multiple providers,
medical, nursing, behavioral health and non-medical staff as
necessary to meet the complete medical socio economic needs of
members
- Monitor referrals made to community-based organizations,
medical care and other services to support the members overall care
management planRequirementsLicensure / Certification /
Registration:
- Registered Nurse obtained prior to hire date or job transfer
date required. Licensure required relevant to state in which work
is performed.
- BLS Provider preferred.
- Case Manager credentialed from the Commission for Case Manager
Certification (CCMC) preferred. Licensure required relevant to
state in which work is performed.Education:
- Diploma from an accredited school/college of nursing
required.Additional Preferences
- BSN or MSN preferred
- Must have a Compact RN License
- Obstetric Case Management experience strongly preferred
- 3+ years of clinical practice as a Registered Nurse strongly
preferred
- 1+ years of case management in a managed care setting strongly
preferred
- Background in managing populations with complex medical or
behavioral needs strongly preferred
- Bilingual (English & Spanish) preferred
