The RN Manager, Care Management is responsible for oversight of the CIN Care Management interventions, strategy and performance management. This oversight includes positive engagement of the team, productivity and metric attainment, clinically appropriate progression of patient experience, audits, training, regional coordination and overall growth and development of the teams that they support.
If you are located in Texas, you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:
Directly lead a team of Care Managers supporting Clinically Integrated Network patients
Directly lead engagement efforts for the entire Care Management team
Attend and actively participate in regional meetings, presentations, or other required meetings
Process, interpret and identify any trends via reports to contribute to new interventions, care plans or other process improvement activities
Serve as primary point of contact for any escalated member, provider or regional issues or concerns.
Perform 1-1s with direct reports at least weekly for the first 90 days and then at least once a month or delegate appropriately to Supervisor
Monitor reports and performance at the plan level for clinical, utilization and financial outcomes, creating strategy for new interventions as necessary based upon metrics
Monitor reports and develop insights for department trends on patient satisfaction and complaints trends and opportunities
Perform supervisory activities related to time management and resource approvals for the team. This includes timesheet approval, approval of PTO, approval and tracking of OT
Engage in performance management actively with direct reports. This includes coaching for success with creation of annual performance goals as well as individual development plans and at least quarterly conversations about progress to said goals. In addition, it may also include coaching for improvement by the timely use of Corrective Action Plans
Participate in at least three process improvement activities for the department annually
Perform chart audits as assigned
Other duties as assigned

Required Qualifications:
Graduate from accredited Bachelors' Registered Nurse program Current unrestricted Texas Registered Nurse License
Case Management Certification (CCM) - within 9 months of hire
2+ years supervisory/management experience
2+ years case management experience
Preferred Qualifications:
Masters' level education completion
Physical Demands & Working Conditions:
Ability to withstand prolonged standing and walking with the ability to move or lift at least 50 pounds.
